Permission, Privacy, and Safety

We know what a great responsibility it is to care for and teach your children, and want to do everything possible to ensure their safety while they are entrusted to our care. Completing our Permission, Privacy, and Safety Forms helps us by providing contact, allergy, and other needed information.

Permission Forms

Permission Forms are needed for certain activities, generally those which involve your children leaving the church property. Currently, there are no children's events requiring a permission form; when permission is required for an event it will be posted here or on the corresponding event page.

Privacy

Our Privacy Policy states in part that New Hope Church publishes photographs and other information regarding church activities on its website, in its newsletter, in informational brochures, in local newspapers, and denominational publications regarding church activities. We respect your children's and family's privacy and will not publish photos or other information without your permission. To grant New Hope Church permission to use photos, videos, and/or audio recordings of your children for publicity purposes, please print and complete this permission form. Safety

When your children participate in classes or activities at New Hope, we ask that you provide current contact, allergy, and medical information, as well as any other information that will help us provide your children with the best possible experience. A Safety Form should be completed at least once a year. You can complete or update this form at any time, and we'll contact you annually for updates.
